What to do next

  • When you create your will or living trust, include the appropriate sample bequest language below.
  • If you already have a will or living trust, have your attorney add the appropriate language in a codicil, or amendment.
  • Be sure to include how you would like your gift to be used. Please contact the Office of Gift Planning to learn more about designating your gift.

Sample Bequest Language

"I hereby give to the Lucile Packard Foundation for Children's Health*, a California not-for-profit 501(c)(3) corporation (tax ID# 77-0440090), the following described property: (description of the dollar amount, portion of estate, or property) to be bequeathed to Packard Children's Hospital."

* Lucile Packard Foundation for Children's Health, an independent 501(c)(3) not-for-profit corporation, fundraises for Lucile Packard Children's Hospital and the pediatric programs at Stanford University School of Medicine. Click for more information.
